In a home emergency, EMT-ALERT will speak for you and your family members in your absence or if you are unable to speak for yourself. The EMT-ALERT can easily be removed from the fridge by a family member or emergency personnel and taken with the patient to the hospital for quick reference to important family and medical information. The information on the front of each EMT-ALERT is there for emergencies of any kind. No home with children, elderly family members, baby-sitters or care-givers should be without one.

We have created a way for you to print out your own EMT-ALERT, at home, free of charge, using your own computer, printer and an ordinary sheet of white 8½" x 11" paper. If you would like, you can purchase one of our 4" x 6" magnetic holders to display your free EMT-ALERT on your refrigerator. The point is, it is FREE, so print one and use it! It could save your life or the life of a loved one.

The EMT-ALERT is to be printed on both sides of an ordinary sheet of white 8½" x 11" paper. You can choose to print a version with a living will or without one. Click on the links below for the version you wish to print. The EMT-ALERT requires Adobe Reader to view and print. You can download it for free if necessary.

  1. Print page #1, with or without a living will, then turn the printed paper over and reinsert it into your printer.
  2. Print page #2 on the back of page #1.
  3. Trim off the scrap paper along the one side of page #1 as shown.
  4. Fill in the necessary information with pencil so it can be easily updated later.
  5. Fold the sheet on the dotted lines leaving the red and black title page on the outside.
  6. Slide the completed EMT-ALERT into any of our 4" x 6" pocket style magnetic frames.
  7. Display your EMT-ALERT in plain sight magnetically, on your kitchen fridge door.
